MyPennMedicine Medical Record Access Terms and Conditions
Acceptance of Terms and Conditions is required to access your Medical information online.
What is MyPennMedicine Medical Record Access?
MyPennMedicine Medical Record Access is an Internet application that enables a patient to have secure web-based access to portions of their University of Pennsylvania Health System ("Health System") electronic medical record, and allows a secure, convenient communications channel with the Health System for non-urgent messages. Use of this service is optional. Your login name and password are key to protecting the confidentiality of your information. It is your responsibility to keep your password confidential. It is only known by you and can be changed online at any time by you. Your password is not known by anyone at the Health System.
Secure Messaging Guidelines and Your Responsibilities:
Please observe the following guidelines for messages sent through MyPennMedicine:
1.) Please DO NOT SHARE your password with anyone.
2.) Please do not send any messages requiring urgent attention. Typical turnaround time for responses is one (1) to two (2) business days.
3.) Please note that myPennMedicine messages may be viewed by other health professionals, administrators and others who work with your physician and his/her practice.
4.) Neither myPennMedicine nor any other application available through this site is a substitute for appropriate and timely contact with your physician. This site is not a substitute for consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her. Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.
5.) Please be aware that not all of your Health System doctors and health care providers use myPennMedicine, so they may not be aware of information that is in your MyPennMedicine. There may also be some information that your doctor believes should not be placed into your myPennMedicine record.
6.) An individual must be age 18 or older to request a personal account to use MyPennMedicine.
7.) You agree that all communication through myPennMedicine will be in regard to your own health condition(s). The contents of any message may be stored in your permanent medical record. Asking for advice on behalf of another person could potentially be harmful to you and is a violation of this Agreement. The Health System does not assume any responsibility for health information or services used by persons other than the myPennMedicine enrollee.
8.) Patients who are users of myPennMedicine should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyPennMedicine. This means that any person with access to a patient's e-mail will be able to see this notification. This could include your spouse, employer or anyone else that can access a patient's e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing myPennMedicine may be information that a patient would not want others to know. Thus, you should take this into account when providing an e-mail address.
9.) By accessing or using myPennMedicine you agree to be bound by all the terms and conditions of this Agreement and the Health System Privacy Statement, and the Health System terms and conditions and disclaimer at https://secure.mypennmedicine.org as posted. We may modify this agreement, the terms and conditions and the privacy statement at any time, and you agree that such modifications are effective immediately upon posting of the modified version.
10.) You agree that myPennMedicine is a communication service offered as a convenience to Health System patients. You understand that it provides you online access to portions of your Health System medical record and that you are solely responsible for any sharing of its content that you intentionally or unintentionally communicate to others. You understand that messages will be routed to the appropriate department as necessary for handling, and therefore Health System employees, other than your healthcare provider may be involved in reviewing or addressing your request. Therefore use of myPennMedicine may not be appropriate for very sensitive information you wish to discuss directly with your provider. You understand that you may receive messages, test results and other communications delivered via myPennMedicine.
For other than general information viewing, myPennMedicine must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).
You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to the Health System by viewing the digital certificate. This certificate verifies the connection between the Health System's server's public key and the server's identification.
Discontinuing Use of myPennMedicine Medical Record Access
You can un-enroll from your use of myPennMedicine Medical Record Access at anytime by completing the appropriate online request to un-enroll or by calling your physician's office.
If we determine at our sole discretion that you have violated this Agreement and/or abused the use of this service, we reserve the right to discontinue your use of this service. You will be notified if we discontinue your myPennMedicine Medical Record Access service.
This Agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania.
